家长委托书、声明书公证按规定必须由法定监护人双方或者不同行监护人本人亲自到公证处办理，不可代办。

For a notarization of a power of attorney and statement from parents, both of legal guardians or any guardian shall proceed with it at the notarization office in person, rather than entrust someone else to proceed with it.

一般需要提交的材料：

Generally the following materials are required:

1、监护人的有效身份证件。

1. The valid ID card of the guardians.

2、监护人和被监护人的《居民户口簿》，集体户籍的申请人提供《常住人口登记卡》本人页原件及经过户籍所在单位盖章的首页复印件。

2. The household register of the guardians and person under guardianship; in the case of an institutional household, the applicant shall provide an original copy of his/her page of Register Card for Permanent Resident and a photocopy of the first page of the institutional register attached with a seal.

3、委托书或声明书格式。不同国家对格式要求有所不同，请向旅行社或该国使馆咨询。

3. Format of power of attorney or statement. Different countries have different requirements on the format. Please consult with the travel agency or the embassy of the concerned country.

4、被监护人的出生证明。可以根据自身情况，从以下四种证明材料中任选其一：①由户籍地派出所或人事档案存放单位出具的出生证明信；②如果您以前办理过出生公证，可以提供原出生公证书；③《出生医学证明》（如果《出生医学证明》内容缺项、涂改、填写字迹不一致，《出生医学证明》不能作为唯一证据使用，还需提供其他证明材料中的任意一项）。

4. Birth certificate of the person under guardianship. Any one from the following three kinds of certificates can be selected as your birth certificate based on your situation: ①The birth certificate letter issued by the police station of your registered residence or the unit where your personnel archives are stored; ②If you ever proceeded with a birth notarization, you can provide your original birth notarization; ③Medical certificate of birth (If your Medical certificate of birth has missed some item, been obliterated or is inconsistent in handwriting, it cannot be used as the only evidence, but shall be used along with any of other evidentiary materials).

5、《独生子女证》可作为辅助证明材料。

1、目的地国家大使馆或组织旅游的旅行社要求办理何种类型公证。未成年人出国旅游经常涉及的公证：孩子出生（实体）、出生医学证明或亲属关系、家长委托书或同意未成年人出国声明。

1. What type of notarization is required by the embassy of the destination country or the travel agency. Notarizations often involved in outbound travel of juveniles include: birth certificate of the child, medical certificate of birth, or statement on family relation, power of attorney of parents or statement agreeing the juvenile to travel abroad.

2、需办理几份公证书；使用国家；公证书需要翻译何种文字；

2. How many notarizations are required; the using country; which language the notarization needs to be translated into;

3、使馆或旅行社对委托书或声明书格式、内容有无特殊要求，例如需要写明去往国家、出行时间等，是否可以提供电子版作为参考，是否允许手写委托书或声明书；

Whether the embassy or travel agency have special requirements on the format and content of power of attorney or statement, like, indicating the destination country, and travel schedule, whether an electronic version can be provided as a reference, whether hand-written power of attorney or statement is accepted;

4、对公证有无其它特殊要求。
